Completing the Requester Tab

After completing the Change Form, complete the Requester tab.  The Requester Tab contains general information about who requested the RFC, who it is requested for, and the change location impacted by the RFC.

By default, your information as the user logged in to Remedy populates the requester fields; however, this can be changed if necessary.  

Complete the sections on this tab by following these steps:

1. Requested By

   1. Enter a First or Last Name of the customer or leave populated if you are the originator of the RFC.

   2. Press the Enter key.

2. Requested For

   1. Enter a First or Last Name of the person the change is requested for.

   2. Press the Enter key.  

If a profile exists in Remedy for the corresponding requester / requested for names, all demographic information will be auto-populated into the appropriate fields in the Requester tab.  If there are multiple matches, a list of names will be provided from which to select.

3. Change Location Information – Select the appropriate location of where the change will take place.  If the default location needs to be changed, select the drop down menus from the fields below.  

   1. Region.

   2. Site Group.

   3. Site.
